• Care Coordination:
• Serve as the primary point of contact for patients and healthcare providers during night shifts, managing and coordinating patient care.
• Ensure timely communication between patients, physicians, and other healthcare providers.
• Assist with scheduling follow-ups, tests, and procedures, ensuring smooth transitions between departments.
• Patient Communication:
• Handle patient inquiries in both English and Spanish, ensuring clear and compassionate communication.
• Provide patient education, clarify treatment plans, and address concerns related to care or services.
• Assist Spanish-speaking patients with understanding healthcare terminology and processes.
• Clinical Documentation:
• Review and update patient records, ensuring all clinical data is accurate and up to date in the electronic health record (EHR) system.
• Coordinate with night shift healthcare providers to ensure proper documentation and handoffs for the next day.
• Administrative Support:
• Handle after-hours administrative tasks, including answering phones, processing patient requests, and triaging urgent clinical matters.
• Work closely with other departments (billing, medical records, etc.) to ensure smooth workflow.
• Compliance and Confidentiality:
• Ensure compliance with HIPAA and other regulatory guidelines related to patient confidentiality and data security.
• Adhere to healthcare policies and procedures to maintain the highest standard of patient care.
• Experience:
• Minimum of 2-3 years of experience in clinical coordination, case management, or a related healthcare role.
• Experience working with U.S. healthcare systems and knowledge of clinical workflows.
• Language Skills:
• Fluent in both Spanish and English (written and spoken) to effectively communicate with bilingual patients.
• Technical Skills:
• Proficient in electronic health record (EHR) systems and other healthcare software.
• Comfortable with night shift schedules and remote communication tools if applicable.
