Key Responsibilities:

  • Assist patients by providing information, coordinating services, and offering support remotely.
  • Conduct outreach to patients for follow-ups, wellness checks, and care coordination.
  • Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems.
  • Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions.
  • Educate patients on managing chronic conditions and adhering to treatment plans.
  • Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
  • Monitor and manage inventory of medical supplies for remote support, if needed.
  • Stay updated with industry best practices in telehealth and chronic care management.

Qualifications:

  • Certified Medical Assistant (CMA) or equivalent.
  • 2-3 years of experience in a medical office, preferably in chronic care management or telehealth.
  • Proficiency in EMR systems and medical terminology.
  • Strong verbal and written communication skills.
  • Bilingual in Spanish highly preferred.
  • Experience working in a remote healthcare setting is a plus.
